CONTROL MEASURES


STEP 1 - Loading unloading & Erection of pipe & material by use of the crane
IDENTIFIED HAZARDS
1 Failure of Equipment & tools 2.Property Damage. Suspended load.  3. Fall, slip, Trip.4.Incompetent Operator 5.Personnel Injury. 6. CONTROL MEASURES
> Re-check the crane with the checklist.
> Lifting plan shall be made in consultation with site engineers/project manager & safety personal.
> Ensure all loads are within the capacity of the crane performing the lifts
> CG of lifted load shall be maintained during shifting of material
> Equipment shall be allowed to work on leveled & consolidated surface.
> Ensure & Verify the correct crane which is suitable for the task.
> Ensure the crane is located  & outriggers are on firm & stable ground
> Outriggers shall be fully extended while using tire mounted crane.
> Pre Task briefing (Toolbox talk) to all involved employees.
> All loads shall be tagged lined during lifting.
> Don’t Cross or walk under the suspended load.
> Ensure Work area demarcated for Authorized personnel only & signs posted for crane operations.
> Ensure proper housekeeping of the work area.
> Loading and unloading of materials should occur at designated dispatch and receivable points
> Crane operators shall have proper ROP driving license &  third party training.
> Ensure operator are well trained in the task and  appoint banks’ men along with the equipment operation
> Ensure the Mobile Equipment, lifting Gears are the third party certified.
> Decide on the method of transport and unloading.
> Always wear proper PPE’s.
> Pre-plan the work, where to Unload, traveling path free from obstruction
> The capacity of the crane shall be decided by a radius vs load system which has mentioned in the load chart.
> Keep visual contact with rigger (pick up & landing points) at all times.
> Once pick/ lift occurs, sound air horn prior to swing
Ensure barricade & signboard fix in the work area
> Barricade the area and provide  signboard 
> Do not lift any loads during wind speed exceeds 10 M/sec
> Stop the lifting Activities during adverse weather conditions.
> Ensure all the fasteners are properly secured inbox
> Ensure that the hand tools are tagged properly.
> Ensure barricade & signboard fix in the work area.
> Ensure no one is working below the area.
> Ensure the ground surface shall be leveled and firmed.
> Ensure no movement in tires while the boom is extended.
> Pre-check the crane with the checklist.
> Ensure all loads are within the capacity of the crane performing the lifts
> Ensure tag lines shall be used unless their use creates an unsafe condition.
> Pre-plan the work, where to position, swing path free from obstruction
Ensure all the riggers are the third party trained for the task.
> Ensure that all Mobile Equipment’s, lifting gears are the third party certified, color-coded as per Site Specification
No overload / beyond the capacity of equipment’s material shall be lifted
> While shifting pipes in the excavated trench at that time nobody can be allowed to stay in the excavated trench.

STEP 2 - Emergency at the work location
IDENTIFIED HAZARDS
> Lack of awareness in case of emergency
> Personal injury
> Fire possibility
CONTROL MEASURES
> Employees must be aware of what to do in case of an emergency.
> All incidents immediately shall be reported to the Site >
> Engineer / Supervisor/ HSE Officer / Project Manager.
> Report to the nearest Assembly point in case any emergency
> Immediately call the emergency contact number.
> Emergency contact numbers shall be displayed near the work area


STEP 3 - Vehicle & Equipment movement, Driving & Transportation inside the plant.
IDENTIFIED HAZARDS
> Fire in equipment during operations Brake failure
> Vehicle / equipment accident
> Personal injury/ death
> Property damage
> Vehicle rollover,
> Splashing of hot water on the driver,
> Burn injury
> Overspeeding
> worn out or nonidentical tires
> Bursting of tires
> Drugs & medicine Weather condition
CONTROL MEASURES
> Operate by ROP licensed operator only
> Check the vehicle and ensure its fitness
> Check tire pressure, brake, speedometer, and fitness of vehicle and stay within the speed limit.
> Enquire the driver health in good condition
> Place fire extinguishers in vehicles/equipment
> Use spark arrestors to avoid fire.
> Obey all traffic rules, signs, and markings.
> Take adequate rests and always follow defensive driving technique, do not use GSM while driving
> Always be alert to face third party mistakes.
> Never drive without being authorized to do so.
> Ensure the driver and passenger wear seat belts.
> Avoid running the engine continuously
> Use hand gloves when touching to the engine
> Vehicles/machines maintenance on a regular basis
> Never overload the vehicle & cover with green nets.
> Check batteries and its condition.
> During an alarm, stop the engine, leave the car in a safe condition, and evacuate to the nearest assembly point.
> Check the weather forecast before starting the work.
> Driver/operator shall check for anybody resting underneath prior to starting the vehicle/plant
> Provide a bank man to guide plant or heavy equipment operations.


STEP 4 - Manual handling of materials
IDENTIFIED HAZARDS
> Back pain and body injury due to poor manual handling.
> Accident due to sharp-edged materials
> Loss of control, slip/trip
CONTROL MEASURES

> All persons must be trained for safe Manual handling.
> Proper PPE shall be used while handling any kind of material.
> Know the weight of all lifting material prior to lifting
>Before carrying heavy wooden/steel material make sure that the root is clear.
> Plan your lift & protect the back.
> Don’t carry more weight than they can carry safely.
> Training on proper posture for manual lifting shall be imparted to all labors
> Impart training on proper lifting procedures.
> Avoid manual handling of heavy material.
> Follow the PTW procedure strictly
> All persons to be made aware of danger through Toolbox talk.
> Intermediate break for the workforce.
STEP 6 -Simultaneous activities
IDENTIFIED HAZARDS
> Hazard unaware 
> Use of improper tools/equipment
> Manual Handling hazards.
> Slips/Trips/Falls injuries
CONTROL MEASURES
> Proper communication should be maintained by both party
> Trained personnel.
> Maintained sufficient distance from other working crews/personnel
> Maintain good housekeeping.


STEP 7 -Working near to the live facilities
IDENTIFIED HAZARDS
> Slips, Trips, Falls
> Skin burn from hot surface etc.
> Electric shock caused by faulty electrical equipment, Cables, etc.
> Cuts and pricks by sharp tools and sharp edges.
> Fire and explosions.
> Release of flammable gases
> Acute/Chronic poisoning due to chemicals.
> Chemical burns, Irritation.
> Mechanical hazards.
> Damage to eyes by splashes of irritating or corrosive materials/ liquids.
> Excessive noise levels.
> Exposure to low or high temperature.
> Musculoskeletal Injuries.
CONTROL MEASURES
> Use adequate PPEs.
> Check electrical equipment by a competent person.
> Verify that all-electric cables are properly insulated.
> Use cut-resistant gloves and Isolation of sharp edges.
> TBT and continuous supervision.
> Wear a respirator or chemical mask, when exposed to harmful dust, vapors, or gases.
> Isolate rotating parts.
> Suitable fire extinguishers.
> Workers should undergo periodic examinations to reveal early possible chronic effects or allergies.
> Use mechanical aids to assist in lifting.
> Use earplug or earmuff.
> Use safe lifting and moving techniques.
> Proper isolation and signage are in a hazardous area.
> All isolation work shall be carried out under the permit to work system.


STEP 8 -Completion of activity, waste management
IDENTIFIED HAZARDS
> Environmental waste. > Improper housekeeping
> Improper waste segregation > Fir
CONTROL MEASURES
> Unwanted Cartridges should be removed from the site.
> Proper waste segregation and Timely disposal to be followed.
> Adequate fire extinguisher at the site.


STEP 9 -Using of Hand & Power Tools
IDENTIFIED HAZARDS
> Electric shock Electrocution.
> Hand/ finger injury
> Cutting / Trapping  Laceration/
> Amputation
> Due to Impact Fracture/ Amputation
> Struck by - Sprain/Strain
> Fracture or Amputation
> Entanglement  - amputation
> Pulled by  - fracture
CONTROL MEASURES
> Electrical equipment must be checked by a competent electrician and color-coded Welding machine, cable & holder shall be inspected & in good condition.
> The only trained person shall use the tools.
> Electrical equipment must be checked by competent electrician and color-coded.
> Check visually before usage.
> Pre task briefing shall be provided prior to start the work.
> Ensure specific personal protective equipment
> Ensure the power tools are double insulated.
> Ensure the workmen's physical fitness for the task.
> Ensure the tools selected are fit for use and suitable for the task.
> Competent workmen shall be engaged in the task


STEP 10 -Radiography of pipe Joint
IDENTIFIED HAZARDS
> Exposure to Radiation
> Injury / Illness / Skin irritation
CONTROL MEASURES
> Unauthorized persons are not present in the radiography area.
> The area shall be barricaded and signage to be posted to identify workplace hazards.
> All radiography activities shall be carried out in compliance with radiation safety requirements.
> Strictly and consistently follow the permit to work procedure prior to starting RT work.
> Conduct a Toolbox meeting prior to starting RT work and brief the concerned the potential hazard and precautionary measure.
> Ensure vehicles carrying radioactive sources should not be parked in crowded areas where other personnel exposed.
> Ensure RT by the competent person only.
> Ensure mandatory PPE’s.


STEP 11 -Working in high Temperature / under direct sunlight
IDENTIFIED HAZARDS
1 Headache 2 Loss of concentration 3 Dehydration
4 collapse of the persons due to heat stress 5 Heatstroke
6 Sunburns 7 Tiredness 8 No Sweating 9 Red or flushed, hot dry skin 10 Rapid pulses 11 Blurred vision 12 Dizziness or fainting 13 Difficulty breathing 14 Unusual behavior
CONTROL MEASURES
> Provide rest shelters
> Heat stress precautionary measures and symptoms of heat stress will be highlighted to all employees in the daily toolbox talks in the summertime.
> All employees will be encouraged to drink sufficient water at frequent interval to compensate the dehydrate
> Drinking water will be provided in the site always
> Drink cool water & avoid drinking cold drinks and alcohol.
> Provide cotton coveralls
> Provision of frequent rest intervals
> All workers will be advised to take a short break if feeling fatigued
> Heat exhausted person will be immediately attended the first aider and will be referred to the hospital if required. 
> Qualified first aiders among workers with first aid box
> Provision of ambulance/ stand by vehicle.
> Direct sunlight works will be suspended during the high temperature and humid.
> Heat stress monitoring and work crew to know limits for working if working inside confined space.
> Prohibited from resting under vehicle/plant.
